These SMS Terms & Conditions (“Terms”) explain how Life Sprout Collective (“Company,” “we,” “our,” or “us”) uses text messaging (SMS) to communicate with you and the rights you have regarding those communications. By providing your mobile number and choosing to receive text messages from us, you agree to these Terms.


1. Purpose of SMS Messaging

We use text messaging solely for administrative, non-promotional, and non-PHI (Protected Health Information) purposes related to your care. Text messages may be sent to:

  • Confirm appointments
  • Coordinate scheduling or rescheduling
  • Notify you of missed appointments
  • Provide closure or delay alerts (e.g., weather-related)
  • Clarify simple, NON-PHI questions related to your availability or visit

We do not send marketing messages or promotional content via SMS.

PHI will never be sent through text messages.
All PHI-related communication must take place through our HIPAA-secure EHR patient portal.


2. When We Collect Your Phone Number

We only request your phone number when:

3. Message Frequency

Message frequency varies based on your appointment activity but typically includes:

  • 1–3 messages per appointment cycle
  • Occasional urgent notifications (e.g., weather closures, rescheduling requests)

We strive to send messages only when necessary to support your care.


4. Opting Out

You may opt out of SMS messages at any time. Simply reply:

STOP — to immediately stop receiving all text messages.
HELP — to receive assistance or contact information.

Opting out of SMS will not affect your ability to receive care.


5. Mobile Carrier Disclosures

  • Message and data rates may apply based on your mobile carrier and plan.
  • Carriers are not liable for delayed, undelivered, or misdirected messages.
  • Delivery of text messages may be affected by your mobile service or coverage area.

6. Data Use & Privacy

Your phone number is used exclusively for the administrative purposes listed in these Terms.

We do not:

  • Sell your phone number
  • Rent or lease your number
  • Share your number with third parties for marketing purposes

7. Protected Health Information (PHI)

To maintain HIPAA compliance:

  • We do not exchange PHI via text message.
  • We will redirect all health-related questions to our secure patient portal.
  • Do not send PHI to us via text.

If you submit PHI through text messaging inadvertently, we will request that future communication be sent through the secure portal.


8. Eligibility

You must be at least 18 years old to opt into SMS communications.
